Summer Student Departure Questionnaire
Dear Summer Students,
The departures will sadly start for some of you at the beginning of August and will continue until the end of September. With this email we would like to inform you about the termination formalities which are also explained at the following link: End of stay formalities | summerstudent.web.cern.ch
On your last Monday you will receive an email with a termination letter attached from the Head of the HR Department in which you will be informed officially of your departure date. You will have to create an EDH termination document by clicking on the link that will be mentioned in the letter. You will have to return all the borrowed articles that are CERNs property (access card, bikes, books, dosimeter…) and ask the respective service to mark the return on your departure form in EDH. If you plan to stay the weekend after your contract has ended at CERN, you can request a visitor pass here. Please note that you cannot keep your access card, you can keep your debit card though as a souvenir.
In addition to the termination letter, you will be asked to:
1. fill out our survey on the summer student programme (below) a great chance for you to give us your feedback and suggestions which we will take into consideration
2. upload your work project report to CDS following the below mentioned instructions. Please ask your supervisor to have a look at the report as it should not contain any confidential information: Project | summerstudent.web.cern.ch
3. fill out any further survey of the lectures and workshops that you attended if not yet done
4. pay your remaining hotel bill
5. if you would like to recommend the programme, we would be happy if you could rate your experience via Glassdoor or Kununu (for German speaking students) and share what you feel makes working at CERN so interesting and exciting
6. if you would like to get an internship attestation, you have to download it from HRT before you leave: Go to HRT and then click on Employment Attestation (if you have problems downloading it, check your browser's Pop-Up blocker settings).
7. come and see us on Friday as indicated in termination email we sent you to approve your departure:

Your CERN computer account and e-mail account will be closed 2 months after the end date of your contract. If you require files related to your work at CERN, then you should discuss this with your supervisor and arrange that the files are copied elsewhere (e.g. to an Internet Service Provider or to a CD or other media).
To transfer the remaining amount on your debit card to your own bank account please submit a Request for Card Balance Refund (attached below) and send it to info@swissbankers.ch.
