Please use the survey below to submit your input for the 2017 annual report. Each project will have a full double page dedicated to it, split into four sections:

A preview of the double-page layout that will be dedicated to each project is available via the PDF in the materials section below.
Before submitting your content, please ensure that it has been agreed (where necessary) with other project members, within your department hierarchy*, and with your collaborators from the companies/research institutes.
* For the IT Department, we will send the final version to the DHO for review.
Please submit your content no later than 14:00 on Monday 13th November, so as to leave time for editing, central CERN review, layout, printing, etc.
Thank you for providing your input for the report. Please note that you can save your responses in the survey at any time (returning later to complete it) before submitting the final version.
Should you, for any reason, prefer not to use the survey tool, please simply add your input in either the Word or PDF template (attached below in 'materials') and send to andrew.purcell@cern.ch.
