The information collected in this form will be used by the HEARTS@CERN selection panel to consider your beam time application for approval, and to collect initial technical information about your test which will help the HEARTS@CERN team with the preparation for your test, with the details to be discussed further ahead in the preparation. All data processed in this form falls under CERN Data Privacy and Protection Rules, collected in Operational Circular 11.
